I declare that the details set forth above are true and correct and the facility will be used for the purposes stated. The hirer of the facility is over 25 years of age and I acknowledge that a security bond of $550.00 is payable and I am responsible for cleaning the facility and removing all rubbish by 10 a.m. the day following. If the facility has not been cleaned satisfactorily and rubbish removed, a cleaning fee of $45.00 per hour plus costs will be charged and deducted from security bond.

I also acknowledge that any damage to the facility will be paid for from the security bond and I am responsible for any costs in excess of the bond.
